Forms & Fees
Payment is required at the time of each appointment. A billing statement with insurance codes will be provided for reimbursement submission. Payment schedules for other professional services will be agreed to when they are requested. Other services include report writing, telephone conversations lasting longer than 15 minutes, consulting with other professionals, with your permission, preparation of records or treatment summaries, and the time spent performing any other requested service.
Once an appointment is scheduled, payment is expected unless you provide 24 hours advance notice of cancellation.
